JUNE 15TH FULL REOPENING DETAILS

Today the California Governor's Office announced new details on the "full reopening" that starts June 15, 2021. There will be no capacity restrictions or physical distancing requirements. California Department of Public Health mask rules must be followed. This means vaccinated individuals do not need to wear masks in most situations, even indoors. Travelers must follow CDD and state travel recommendations. Indoor 'mega events’ will require additional restrictions that can include proof of vaccination or recent negative covid-19 test. The State will keep current masking guidance until June 15, and L.A. County will do the same. This means that LA County Department of Public Health’s guidance remains aligned with the local Health Officer Order of May 3 until further notice.

Workplaces remain under Cal/OSHA masking and distancing requirements, which have not yet been changed. And for unvaccinated people, masks are still required outdoors any time distancing cannot be maintained.

HOW TO OBTAIN A DIGITAL COPY OF YOUR VACCINE RECORD

The Los Angeles County Department of Public Health partnered with Healthvana in December of 2020 to provide a digital copy of the vaccination record to individuals who are vaccinated at Department of Public Health locations, City of L.A. locations, select community clinics (that are not part of a hospital system), pharmacies, or FEMA sites in Los Angeles County. 

If you received a vaccine at one of these locations, you should receive an email from vaccine@Healthvana.com and a text message from Healthvana with a secure link (most text messages will display as 37586). When you click the link in the email or text, you will be asked to enter your first name, last name, and date of birth to confirm your identity. You can then download your record and, if you have an iPhone, put it in your Apple wallet.

Due to the sheer volume of vaccination records being processed, there may be a delay in receiving your digital record from Healthvana. If you don't get the email or text within one month of being vaccinated, visit the healthvana.com webpage and select “contact” in the menu to request your record. Please note that it is not currently possible to upload a self-reported vaccine record into Healthvana unless you already have a Healthvana record of your first dose (see below).

New feature to report 2nd doses for those with a Healthvana record:

Healthvana is offering a new feature that allows you to complete your digital vaccination record by uploading an image of your white CDC vaccination card as proof of your 2nd dose. To do this: 

1.     Log in to your Healthvana account at https://patient.healthvana.com

2.     Navigate to the Vaccinations page

3.     Click the “Add 2nd Dose” button

If you never finished setting up an account, you can do this by following the instructions to set a password here. More information is available on the Healthvana FAQs page. At this time, this feature is only available to those who received a Healthvana digital record of their 1st dose.

Please note, if you received your vaccine outside of the locations listed above and through another medical network, you might not have the ability to access the Healthvana system.  All vaccinated individuals can obtain an official record of their COVID vaccinations from the California Immunization Registry (CAIR). Individuals can ask their primary provider to look up their CAIR records or contact CAIR directly.

What can I do if I lose my white card?

If you did not get a white card, or you lost it, there are two ways to get a copy of your official record from the State's California Immunization Registry (CAIR).

  • Ask your primary care provider to look up your vaccination record in the California state immunization registry (CAIR) and give you a copy.

  • Request a copy of your vaccination record directly from the state immunization registry, CAIR. Please wait at least 2-3 weeks after you are vaccinated before making the request, and know that this request may take an additional 2-3 weeks given the high demand. Complete the online Authorization To Release Form. Have a scan or photo of a current official picture I.D. ready to upload. The form must be completed in a single session. You will receive a record by email. The record can only be requested online and can only be provided by email. For more information, visit cairweb.org.

Can I get a digital vaccination record?

  • The CDC white card or CAIR record is your official proof of your COVID vaccination. You may receive a digital record in addition to your white card as a convenience. Still, it may not be accepted at all places/companies as the official proof of vaccination.

  • Several different kinds of digital reminders and records are being used in L.A. County. The type depends on the record-keeping system used by the provider who administered the vaccine. Most systems require the person being vaccinated to give an email address and/or cell phone number when they make an appointment or get vaccinated. Ask your provider when you get your vaccine about a digital record.

  • The Los Angeles County Department of Public Health (DPH) has partnered with Healthvana to provide a digital copy of the vaccination record to people who are vaccinated at Department of Public Health locations, select community clinics (that are not part of a hospital system), pharmacies, or FEMA sites in Los Angeles County. VACCINE INFORMATION

This weekend at all the County-run vaccination sites, the Department of Health Services vaccination sites, and the City of L.A. vaccination sites, everyone 18 years and older coming to get their first vaccine will have a chance to enter the Los Angeles Lakers 2021-22 Season Ticket Vaccination Sweepstakes to win a pair (2) of season tickets for next season. To be vaccinated at a site run by the Los Angeles County Department of Public Health, you will need to show:

  • Proof of age, and

  • Minors age 12-17 only - Consent form (for more details, see below or visit our website)

You do not need to show government-issued I.D., and you do not need to be a U.S. citizen to get a vaccine. Below is a list of documents that will be accepted. This list is not complete, and other documents may be accepted. One document may meet more than one requirement (e.g., a driver's license).

If you have health insurance, please bring your health insurance card (COVID-19 vaccines are accessible regardless of insurance status)

Examples of proof of age:

  • Driver's license or permit (foreign country or expired I.D. okay)

  • California ID card or REAL ID card (from the DMV)

  • Consular ID (Matricula Consular)

  • Medicare card

  • Military ID

  • Membership card (foreign country okay if written in English)

  • Passport (foreign government or expired okay)

  • Birth certificate

  • Medical document/records (including immunization records) from medical provider, clinic, or doctor

  • Any official document that includes name and date of birth (for example, school record)

Important information for minors being vaccinated at a site run by the Los Angeles County Department of Public Health:

  • A consent form is required for all minors age 12 through 17 at each visit.

  • 16 and 17-year-olds should be accompanied by their parent or legal guardian if possible. If this is not possible, they must bring a consent form signed by their parent or legal guardian.

  • 12 through 15-year-olds must be accompanied by their parent, legal guardian, or a responsible adult. If a responsible adult accompanies the child, the consent form must name the responsible person and be signed by the parent or legal guardian.

  • For all minors, the parent, guardian, or responsible person that accompanies them must bring their photo I.D.

  • Note: If the minor is being vaccinated at school, consent is required; however, the school’s guidance should be followed on whether a parent/legal guardian or named adult needs to be present.

  • Consent forms are available at the vaccination site or can be printed for free in L.A. County libraries.
